That's Joker but what about Two Face, Scarecrow, Riddler, Mad Hatter and Ventriloquist?
I don't think this question can be applied to all batman villain though Killer Croc may or may not wear underpants I doubt Clayface needs them and as for Ra's a Ghoul probally neither boxers nor breifs but some kind of underwear that hasen't been used by the general public for years
